Job description

ASZ Caring Hearts — Physical Therapists (PT), Physical Therapist Assistants (PTA), Occupational Therapists (OT), Occupational Therapy Assistants (OTA), and Speech-Language Pathologists (SLP) — Managed Care Home Visits

Join ASZ Caring Hearts, one of the largest privately owned Home Health Agencies in Southeast Florida. We’re seeking experienced, compassionate PTs, PTAs, OTs, OTAs, and SLPs to provide high-quality managed care home visits. If you thrive in an interdisciplinary team, put patients first, and value autonomy with strong clinical standards, we’d love to meet you.

What you’ll do

  • Provide evidence-based therapy in patients’ homes for managed care plans, focused on safety, function, and goals.
  • Complete plan-specific evaluations, treatments, progress notes, and reassessments on time.
  • Collaborate with nursing, case managers, and other disciplines to optimize outcomes and satisfaction.
  • Educate patients and caregivers on home programs, equipment use, communication/cognitive strategies (SLP), ADL/IADL training (OT/OTA), and fall prevention.

What we’re looking for

  • Current, active, and clear Florida license (PT, PTA, OT, OTA, or SLP).
  • 1+ year home health experience preferred.
  • Knowledge of Florida and payer-specific home health regulations and documentation.
  • Bilingual a plus — English/Spanish or Creole.
  • Reliable transportation; comfortable working “on the road.”
  • Compassionate, professional, and team-oriented.

Schedule and employment type

  • Visit-based roles with flexible scheduling.
  • PRN, Part-time, and Full-time opportunities available.

Compensation

  • Visit-based pay for managed care cases.
  • Typical range: $75.00 – $90.00 per hour equivalent, depending on discipline, visit type, and payer.
