Key Responsibilities
  • Personal & Hands-On Care: Assist clients with essential daily routines, including bathing, dressing, grooming, oral care, and personal hygiene.
  • Mobility & Safety: Help clients transfer safely, ambulate around their home, and navigate their space to prevent falls and injuries. 
  • Daily Living Support: Prepare nutritious, well- balanced meals, assist with light housekeeping (tidying, laundry, dishes), and help keep the living space safe and organized. 
  • Companionship & Connection: Build meaningful, respectful relationships with clients through engaging conversation, emotional support, and shared hobbies. 
  • Health Observation & Reporting: Keep track of daily routines, note subtle shifts in physical condition or behavior, and communicate updates clearly to our care coordination team. 
  • Errands & Transportation: Run local errands, assist with grocery shopping, and provide reliable transportation to appointments or community outings. 
Requirements
  • Current New Hampshire Licensed Nursing Assistant (LNA) or Personal Care Aide (PCA) certification preferred
  •  Required previous experience in home care, assisted living, or another clinical caregiving environment 
  • Dementia Care Certification is a plus 
  • Valid driver’s license and reliable transportation
  • Excellent communication skills with a compassionate, patient, and respectful approach to care
  • Ability to work independently, prioritize responsibilities, and manage time effectively in a home care setting
  • Successful completion of a background check and compliance with all applicable state requirements
