Job description

Essen Health Care is the largest privately held, multispecialty medical group in New York, providing high-quality, compassionate care to some of the state’s most vulnerable and underserved residents.

Founded in 1999, we’ve grown from a single primary care office into a network of 50+ locations offering urgent care, primary care and specialty services, from women’s health to endocrinology and psychiatry. We also provide nursing home support, care management, and in-home care through our Essen House Calls program. Guided by a Population Health model, our team of 500+ providers deliver care in-person, at home, or via telehealth, ensuring patients get the support they need when and where they need it.

Our Program Coordinator will work 50% of the time with the NYREACH team to facilitate and develop workforce development initiatives as needed, create schedules, oversee progress and ensure goals are met on time. He/she is also responsible for coordinating and attending meetings, developing project strategies, operating our WFD program, and providing all administrative assistance as required.  

For the other half of the job, our Project Coordinator will be responsible for finding and developing grant opportunities that align with NYREACH programming and objectives. 


  • Coordinate project management activities, resources, equipment and information 
  • Break projects into doable actions and set timeframes 
  • Act as interdepartmental liaison to identify and define requirements, scope and objectives for assigned projects.  
  • Assist with schedule management and outreach for WFD programs. 
  • Adapt to the dynamic needs of project/program management.  
  • Analyze risks and opportunities 
  • Monitor project progress and handle any issues that arise 
  • Act as the point of contact and communicate project status to all participants 
  • Use tools to monitor working hours, plans and expenditures 
  • Create and maintain comprehensive project documentation, plans and reports 
  • Ensure standards and requirements are met through quality assurance tests 
  • Network with other organizations as related to WFD opportunities  
  • Provide administrative assistance to your direct supervisor 
  • Research grant opportunities and develop applications 
  • Design, develop, and deliver trainings 

  • Proven work experience as a Program Coordinator or similar role 
  • Proven experience as a grant writer 
  • Experience in project management, from conception to delivery 
  • An ability to prepare and interpret workflows, schedules and step-by-step action plans 
  • Solid organizational skills, including multitasking and time-management 
  • Strong client-facing and teamwork skills—incl. communication and leadership 
  • Familiarity with risk management and quality assurance control 
  • Strong working knowledge of common Microsoft Office applications, especially Excel, Word, PowePoint and Visio as well as Canva. 
  • Excellent communication and interpersonal skills, with the ability to effectively collaborate with diverse teams and stakeholders.  
  • Detail-oriented and highly organized 
  • Bachelors Degree – Public Health, Health Services Administration or related field.
