Location: Albany, OR
Schedule: Full-Time | 40 Hours per Week
Pay: $50.00 - $56.00 per Hour

Position Summary
As a Home Health Occupational Therapist, you'll evaluate patients' functional abilities, develop customized treatment plans, and provide therapeutic interventions that help individuals regain independence and improve their daily living skills. You'll work collaboratively with physicians, caregivers, and an interdisciplinary care team to deliver exceptional outcomes.
Essential Responsibilities
  • Evaluate patients and develop individualized treatment plans based on physician orders
  • Provide occupational therapy services in accordance with established care plans
  • Educate patients, families, and caregivers on treatment strategies and adaptive techniques
  • Assess home environments and recommend safety modifications and assistive equipment
  • Maintain accurate and timely clinical documentation
  • Collaborate with physicians and interdisciplinary team members to ensure optimal patient outcomes

Qualifications
  • Current Occupational Therapist license in the State of Oregon (or eligible for licensure)
  • Graduate of an accredited Occupational Therapy program
  • Home health experience preferred
  • Strong communication, organizational, and time-management skills
  • Ability to work independently
  • Valid driver's license, current auto insurance, and reliable transportation
