This position is primarily remote, allowing for flexibility while maintaining close collaboration with the Hospice team. • Conduct comprehensive psychosocial assessments of patients to evaluate their mental health, strengths, and needs, tailoring interventions accordingly.
• Implement evidence-based therapeutic modalities, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), addressing varied patient needs.
• Collaborate effectively with the multidisciplinary care team to ensure a coordinated approach to patient care, participating in team meetings and case discussions.
• Educate patients and their families about diagnoses, treatment options, and potential medication impacts to foster informed decision-making.
• Facilitate individual, group, and family therapy sessions using evidence-based practices that focus on the patient's unique circumstances.
• Develop and periodically review individualized treatment plans that incorporate patient goals, preferences, and clinical insights from the care team.
• Assist patients with navigating the complexities of the mental health system, serving as an advocate for their rights and addressing their unique needs.
• Lead psychoeducation sessions and therapeutic groups to empower patients and families, focusing on coping strategies and resources available in the community.
• Participate actively in discharge planning meetings, ensuring smooth transitions for patients moving out of hospice care.
