Job Summary and Qualifications

We are seeking a Lead Registered Nurse Case Manager. You will provide supervision of the Case Management staff, ensuring delivery of quality case management services and contribute to the continuity of services and effective operation of the department.

 In this role you will:

  • Develop, initiate, and coordinate individual patient case management plans to ensure appropriate level of care across the continuum.
  • Ensure plan meet the patients’ clinical, psychological and discharge needs in collaboration with the attending physician and interdisciplinary team.
  • Assess the health status of assigned patients by collecting and analyzing patient and family data, and expedite and coordinate the delivery of services to facilitate patient’s progression through the healthcare system.
  • Perform admission certification screening for patients
  • Monitor and control the use of healthcare resources to achieve desired patient outcomes, decrease length of stay, and decrease resource utilization. 
What qualifications you will need:
  • Must be licensed as a Registered Nurse in accordance with state regulations
  • BSN Preferred
  • American Heart Association BLS
  • 2 years medical surgical experience required
  • Previous Case Management experience preferred
  • Certified Case Manager (CCM) preferred
