• Direct and manage individualized physical therapy treatment plans aimed at preventing disability, alleviating pain, and enhancing muscle function in accordance with physician prescriptions.
• Assess client progress through comprehensive evaluations that incorporate diagnostic and prognostic tests to determine muscle, nerve, joint, and functional capabilities.
• Collaborate closely with Physical Therapy Assistants to ensure the effective implementation of treatment plans, providing guidance and support as necessary.
• Evaluate patient home environments to make informed recommendations promoting rehabilitation and independence.
• Administer a range of therapeutic services, including manual exercises, patient education, and physical agent modalities like traction and massage.
• Fit and adjust prosthetics and orthotics while communicating modifications to supervising physicians.
• Document clinical findings in the client’s file, prepare detailed progress notes, care plans, and discharge summaries in compliance with OASIS requirements.
• Actively participate as a member of interdisciplinary teams, combining efforts with other healthcare professionals to optimize treatment outcomes.
• Engage with clients and their families, educating them on therapy processes and the correct use of rehabilitative devices.
• Uphold administrative responsibilities by submitting timely documentation, orders, and reports in line with regulatory requirements.

Our outpatient services cater to a diverse population, primarily focusing on orthopedic patients requiring rehabilitation after surgery, injury, or chronic pain management. • Bachelor’s Degree in Physical Therapy from an accredited program.
• Active state licensure in physical therapy, maintaining good standing.
• Current CPR certification and a valid driver’s license along with automobile insurance.

• Strong interpersonal communication skills to effectively engage with patients and families.
• Ability to collaborate with team members and lead initiatives that enhance patient experience.
