Sentara Health is hiring for a Behavioral Health Navigator III at Sentara Careplex in Hampton, VA!
Status: Full-time, permanent position (40 hours)
Standard working hours: 8am to 5pm EST, M-F
Location: Will report daily at Sentara Careplex in Hampton, VA!
Overview
The behavioral health navigator level III serves as a senior member of the care coordination team, providing expert support for high-acuity behavioral health cases. This role includes workflow leadership, mentoring, and project participation to enhance service delivery across the continuum.
Education
  • Master's Degree in one of the following areas: Social Work, Counseling, Clinical Mental Health Counseling, Marriage and Family Therapy, Psychology, Addiction Studies, Nursing or a closely related field OR Bachelor's of Science in Nursing for RN Candidates
    REQUIRED

Certification/Licensure:
  • Basic Life Support (BLS) - Certification - American Heart Association (AHA) RQI required within 90 days of hire.

Experience
  • Eight years of healthcare experience delivered in one of the following settings: Acute care, long-term care, home health, hospice, skilled nursing facility, or community-based program services REQUIRED
