Work Environment

This position will primarily be remote, allowing for flexible scheduling with occasional face-to-face interaction with clients as needed. Standard work hours are Monday through Friday from 8:00 AM to 4:30 PM, although some evening or weekend hours may arise based on client requirements. Adaptability is key, as our social worker will need to respond quickly to emergent situations or provide support during off-hours as required. The remote setting allows our social worker to connect with clients in their own environments, fostering comfort and open communication.

Key Responsibilities
  • Conduct comprehensive assessments of seniors' psychosocial needs, taking into account their emotional, social, and mental health factors.
  • Collaborate closely with clients and their families to formulate personalized care plans that align with their specific goals, preferences, and life circumstances.
  • Offer counseling, therapeutic interventions, and emotional support to seniors and their families during life transitions such as health declines or loss of independence.
  • Act as an advocate for clients, navigating them through complex healthcare systems and ensuring they have access to vital community resources and services.
  • Engage with interdisciplinary teams, including healthcare professionals, caregivers, and nurses, to facilitate a coordinated approach to each client's care.
  • Conduct regular check-ins with clients to monitor progress, evaluate living conditions, and proactively address safety concerns or emergent issues.
  • Resolve conflicts and facilitate effective communication among family members involved in the care of senior clients.
  • Educate both clients and their families about available support groups, community resources, and programs to enhance social engagement and quality of life.
  • Maintain accurate and compliant documentation regarding client assessments, care plans, and interactions to ensure continuity of care and adherence to ethical standards.
  • Stay updated on the latest social work practices and policies while ensuring compliance with relevant laws and ethical guidelines.
Patient Population / Client Focus

This position focuses primarily on supporting the senior population within the Rochester community. The ideal candidate will possess extensive knowledge and understanding of aging-related issues, including mental health concerns unique to this demographic. Qualifications
  • Must hold a Masterโ€™s degree in Social Work from an accredited institution.
