You will collaborate closely with an interdisciplinary team of healthcare professionals, leveraging technology and communication tools to ensure efficient and effective service delivery.

• Collaborate with an interdisciplinary team to create and enhance individualized care plans that address the unique needs of each patient.
• Provide comprehensive support and information to patients and families regarding transition plans, guiding them through changes in their healthcare journey.
• Maintain up-to-date knowledge of government and private payer networks to facilitate appropriate care transitions.
• Engage with community resources and healthcare agencies to ensure continuity of care for patients, enhancing access to necessary services.
• Advocate for patient rights, ensuring access to protective services and promoting awareness of ethical standards in mental healthcare.
• Monitor and report potential cases of vulnerability or suspected abuse, adhering to legal mandates and ethical responsibilities.
• Participate in care systems aimed at preventing patient readmissions by identifying at-risk patients and utilizing clinical tools effectively.
• Plan and lead transition conferences with patients and their families, ensuring clear communication and coordinated care plans.
• Utilize electronic medical record systems to track patient progress, document interactions, and communicate updates with the care team efficiently.
• Other responsibilities as assigned to support overall team objectives and patient care quality.

The primary focus of this position is on patients experiencing life-threatening, chronic, or acute health conditions. You will work closely with diverse populations, including families navigating complex health issues, ensuring they receive the necessary psychosocial support throughout their treatment and recovery journey.

• An independent license in the state of Oregon (LCSW, LMFT, or LPC) is required.
• A Master's or Doctorate degree in clinical psychology, counseling, or clinical social work from an accredited institution.
• A strong foundation in using electronic medical records for documenting and monitoring patient progress and care plans.
• Exceptional email communication skills to facilitate collaboration across multidisciplinary teams.
• Proven ability to manage a large caseload effectively while maintaining a focus on quality care.

• Demonstrated passion for improving behavioral health outcomes.
• Experience in advocating for patient rights and navigating healthcare systems.
• Familiarity with community resources and healthcare services available in Oregon.
