Job Description

Your Impact:

  • Deliver Field-Based Care:Conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home.
  • Manage Chronic Disease:Create longitudinal care plans that move the needle for high-risk patients.
  • Collaborate Across the Continuum:Partner with an interdisciplinary team to ensure seamless, safe care transitions.
  • Prevent ER Visits:Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.
Job Requirements

What You Bring:

  • Advanced Degree:Master’s Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family.
  • State Required Credentials:Active, unencumbered NP license and Board Certification (ANCC or AANP).
  • Reliable Mobility:Valid driver’s license and vehicle for local field-based travel.
