Position Summary
The LPN / Licensed Practical Nurse plays a vital role in supporting the health, wellness, and quality of life of residents within our Assisted Living and/or Memory Care communities.
This position is responsible for resident assessments, care planning support, medication and treatment management, clinical documentation, and ongoing communication with residents, families, physicians, and caregiving teams.
The ideal candidate is compassionate, clinically skilled, organized, and passionate about enhancing the lives of seniors through exceptional care and teamwork.
Responsibilities
  • Provide nursing care and clinical support to residents
  • Assist with resident assessments and care planning processes
  • Administer medications and treatments according to physician orders and company policies
  • Monitor resident conditions and communicate changes appropriately
  • Maintain accurate clinical documentation and medication records
  • Partner with caregivers, medication technicians, physicians, and families to ensure quality resident care
  • Support resident dignity, comfort, and quality of life
  • Assist with compliance, infection control, and quality assurance processes
  • Support and mentor caregiving staff as needed
  • Foster positive relationships with residents, families, and team members

Qualifications
