Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

What You'll Be Doing

• Provide advanced wound care services, treatment planning, and ongoing management of complex wounds.

• Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long-term care communities.

• Manage medications, diagnostics, treatment plans, and follow-up care.

• Collaborate closely with physicians, nurses, therapists, caregivers, and facility leadership.

• Support preventative care initiatives and continuity of care efforts.

• Help improve healing outcomes while reducing avoidable complications and hospitalizations.

Qualifications

• Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred).

• Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing preferred.

• Strong clinical judgment and confidence managing medically complex patient populations.

• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
