As a Nurse Practitioner (NP) or Physician Assistant (PA) at CenterWell Senior Primary Care, you will be part of a collaborative, diverse team. Your Role & Responsibilities

  • You will provide comprehensive care to seniors following clinical standards.
  • You will collaborate daily with your care team to ensure coordinated, high-quality care.
  • You will manage referrals, hospital/SNF coordination, DME, and home health services.
  • You will document care with support from quality-based coders.
  • You will participate in on-call rotation and support growth plans at your center.

What We're Looking For

  • You will have a Master's Degree in Nursing or completion of a PA program with board certification.
  • You will have an active, unrestricted NP or PA license in the state of practice.
  • If selected for this role, we will require you to screen for TB.
