JOB TITLE: Developmental Disability Service Coordinator

LOCATION: Boardman, OR - Crisis Building

FLSA: 1.0 FTE, Non-Exempt (40 hours a week Monday through Friday)

SUPERVISOR: DD Site Supervisor/DD Program Manager

PAY GRADE: B9 ($27.55 - $40.58 hourly, depending on experience)

DESCRIPTION


Provides case management services to individuals in the Developmental Disabilities program in Morrow County in accordance with the Oregon Administrative Rules. Coordinates other services and activities within the Developmental Disability Program, and performs other tasks as identified by the DD Site Supervisor, DD Program Manager, or Chief Operating Officer. This position requires traveling within the County to meet the needs of the individuals enrolled in the program. An agency vehicle will be provided.

Works under the direction of the DD Site Supervisor, Program Manager, or Chief Operating Officer.

Supervision Exercised

This position does not supervise other employees.


RESPONSIBILITIES

  • Assess the eligibility of potential clients utilizing guidelines as provided by the Oregon Administrative Rules.
  • Provide case management services to all customers. Services are provided in a variety of community settings, with consideration to client preference.
  • Maintenance, submission, and timeliness of all records, case notes, reports, and all other required documentation.
  • Assist customers in acquiring services and resources needed to achieve goals and maintain independence in the community.
  • Coordinate with other community partners to develop, provide, and maintain a rich array of services and resources for clients.
  • Provide transportation for customers when required.
  • Attend local, regional, and state meetings as requested by the supervisor.
  • Act independently within the community and promote a positive image of the agency. Participate in all team and agency meetings as scheduled.
  • Keep updated on all Individual Service Plan (ISP) meetings and established objectives.
  • Meet all billable services contact requirements during the month, unless otherwise arranged through the Executive Director.
  • Investigate any reports of abuse and implement protective services for all customers when required.
  • Function as a broker for services to ensure customers meet necessary contacts (e.g., psychiatrist, medical doctor, Supplemental Social Security Insurance, etc.) as required.
  • Participate in joint case planning and coordination with other CCS employees.
  • Remain updated on all rules and Oregon Administrative Rules (OAR's) pertaining to Developmental Disabilities (DD) Services.
  • For contractors and providers that CCS is required to monitor, ensure that they adhere to relevant OAR's.
  • Provide consultation to on-call staff supporting individuals who experience a developmental disability.
  • Input all necessary information into CCS databases for clients receiving services in the DD program.
  • Assist DD Program Manager with the recruitment and maintenance of Lake County Advisory board members.
  • Other responsibilities as assigned.

Requirements

QUALIFICATIONS


Education and/or Experience

Must meet the minimum standards for a Service Coordinator/Case Manager under Community Developmental Disability Programs as defined in the Oregon Administrative Rule (OAR) 411-320- 0030 and 411-415-0040

a) A bachelor's degree in behavioral science, social science, or a closely related field; OR

b) A bachelor's degree in any field and one year of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing; OR

c) An associate's degree in a behavioral science, social science, or a closely related field and two years of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing; OR

d) Three (3) years of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing: OR

e) If a candidate does not meet the above education/experience requirements Community Counseling Solutions CDDP may, at its sole discretion, submit a request for a variance to the Office of Developmental Disabilities.

Other Skills and Abilities

Must know, or be able to learn, DD case management standards and the ability to implement them within the communities served. Must have knowledge of, or the ability to learn about, DD services, resources, and funding sources.

Strong communicator with demonstrated ability to effectively communicate, both verbally and in writing.

Work independently and complete all responsibilities in a thorough, timely fashion.

Must have the ability to represent the interests of the customer and the agency in a favorable light to the community.

Must hold a valid drivers license and be insurable or carry insurance for client service purposes and for travel between business offices, the community, and to attend required meetings and/or trainings.

Must be computer literate and utilize word processing and other software programs/systems.

The position requires the handling of highly confidential information. Must adhere to rules and laws pertaining to client confidentiality.


CRIMINAL BACKGROUND CHECKS


Must pass all criminal history check requirements as required by ORS 181.536-181.537 and in accordance with OAR 410-007-0200 through 410-007-0380.

In addition to a pre-employment background check, each employee, volunteer, and contractor shall be checked monthly against the OIG and GSA exclusion lists, as well as other federal and state agency lists. If it is discovered that an employee, volunteer, or contractor is excluded or sanctioned it will be grounds for immediate termination of employment, volunteering, or the termination of the contract.


PRE HIRE DRUG SCREEN REQUIRED


PERSONAL AUTO INSURANCE


Must hold a valid driver's license as well as personal auto insurance for privately owned vehicles utilized for CCS business such as client service purposes, travel between business offices and the community, to attend required meetings and trainings.

Must show proof of $300,000 or more liability coverage for bodily injury and $100,000 or more in property damage and maintain said level of coverage for the duration of employment at CCS.

The employee's insurance is primary with CCS insurance being secondary. CCS reserves the right to deny any employee the use of a vehicle owned by CCS.


PHYSICAL DEMANDS


While performing the essential duties of this job, the employee is regularly required to use office automation including computer and phone system that requires fine manipulation, grasping, typing, and reaching.

The employee is also regularly required to sit, talk, listen, and use hands and fingers. The employee is also required to stand, walk, reach with hands and arms; stoop; kneel and/or squat.

The employee may occasionally lift and/or move up to thirty pounds. Specific vision required by this position includes close, peripheral, distance, and the ability to adjust focus.

This work also requires that the employee sustain their attention during periods of crisis.

WORK ENVIRONMENT


Tasks will occur both at the office and in the community. There will be travel required throughout the county(s) that the employee is assigned to. Frequent travel within the County will be required for this position. Additionally, out-of-area travel and overnight stays will be required for attendance at meetings and/or trainings. Monitor and broker for services at the home site of the customer, service provider, and office will be within a typical day's routine.

As indicated, employee also will be required to work in the community and homes of the customers we serve. Handicap access may not be available at all places where this position must go. This position may be required to respond to environments where a customer is in crisis. The environments in these situations are difficult to predict and may be in uneven terrain.

This position may be exposed to the everyday risks or discomforts which require normal safety precautions typical of such places as an office.
