Job Title: Registered Nurse Case Manager

Experience: One year of experience as a clinical nurse in a hospital required, and 9 months experience in home care preferred.

License: Current RN License (NY License ONLY)

Certifications: BLS, OASIS, Telemetry

Must-Have: Certified Home Health Care Experience, EPIC or Netsmart experience required

Description: The Registered Nurse Case Manager develops and revises treatment and discharge plans in conjunction with the patient, family, physician, care delivery team, and third-party payors. Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition within defined time frames. Processes orders and notifies the physician of patient needs and changes in condition. Completes certification and recertification orders and discharge summaries. Provides an environment that promotes respect for patients, their privacy, confidentiality, and property. Initiates appropriate preventive and rehabilitative strategies. Provides services that require substantial specialized nursing skills. Consults with and educates the patient, family, and other team members regarding disease processes, self-care techniques, and preventive strategies. Supervises LPNs to ensure patient health, safety, and compliance with the plan of care. Directs, coordinates, evaluates, and supervises the quality of patient care services. Interacts with patients, physicians, referral sources, and others in a manner that fosters positive relationships. Supports Care Management philosophy through collaborative customer-focused planning and case management. Ensures compliance with state and federal legislation in all clinical records. Maintains current knowledge of community-based services. Demonstrates flexibility and willingness to visit patients based on need. Ensures problem resolution at the point of contact. Attends patient care conferences for patients assigned. Monitors results and progress related to quality indicators, including customer satisfaction. Understands the impact of the RN function on patients and overall care. Pursues efforts to reduce or eliminate avoidable costs and errors. Responds to findings from various quality management reviews and ensures resolution of potential issues. Participates in peer review and quality assessment as assigned. Maintains productivity requirements. Ensures visit authorizations are in place and that documentation reflects medical necessity for home care. Promotes respect and celebration of workforce diversity. Participates in educational programs and self-development initiatives. Demonstrates commitment to professional growth and competency. Responsible for Key Performance Indicators (KPIs) established by management. Performs additional duties as requested.
