Position Overview

Jackson Therapy Partners is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our remote team. In this full-time role, you will be instrumental in providing therapeutic and counseling services to residents of a hospice center. Key Responsibilities
  • Conduct thorough assessments of patients' psychosocial needs and capabilities to create effective treatment plans.
  • Deliver individual, family, couple, and group psychotherapy sessions tailored to the unique needs of our residents.
  • Collaborate actively with the psychiatry team and other healthcare professionals to integrate treatment planning and programming.
  • Evaluate and monitor the ongoing biopsychosocial needs of patients, adapting treatment strategies as necessary based on their evolving conditions.
  • Provide case management and psychotherapy services to assigned patients, ensuring a holistic approach to their care.
  • Maintain comprehensive progress notes using electronic health record systems; training will be provided to facilitate this process.
  • Engage in continued professional development, contributing to team meetings and case discussions.
Patient Population / Client Focus

The primary focus of this role is to support elderly patients in hospice care who may be facing various psychosocial challenges as they approach the end of life. You will ensure that their emotional and psychological needs are met with sensitivity and compassion, fostering a supportive environment for both patients and their families during this sensitive time.

Qualifications
  • A valid Licensed Clinical Social Worker (LCSW) certification in Pennsylvania is required.
  • Possess or be willing to obtain professional liability insurance to practice independently.
  • Experience or willingness to work specifically with senior populations in hospice or similar environments.
  • Strong ability to document and track patient progress through electronic health record systems; training will be provided.
  • Ability to collaborate effectively with a diverse team, as well as work autonomously.
  • A high level of integrity, reliability, and adaptability, demonstrating a commitment to ethical practice.
Preferred Experience
  • Familiarity with Medicare provider guidelines is preferred but not mandatory.
  • Experience creating and implementing therapeutic interventions within a group or family therapy context is beneficial.
  • Proficiency in utilizing telehealth platforms for conducting therapy sessions is advantageous.
