  1. Complete this Volunteer Application (attach identification, TB test or TB Risk Assessment form and mandated reporter training)
  2. Complete Fingerprint Clearance and Return a Signed Request for Live Scan Service Form (no cost options to volunteers when using: Fingerprinting Options - Please call or email for appointment - Diablo Livescan 925-754-3100 or diablolivescan@yahoo.com)
  3. Once we receive your application, you will receive an email from Keenan, our compliance platform, with a link to complete the required Mandated Reporter Training.
