Description

Supports the Mission, Values and Vision of Lydia Care Center and the facility. 

Is knowledgeable of consumer rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all consumers in a safe, secure environment. 

Supports, cooperates with, and implements specific procedures and programs as outlined within the facility handbook, on-going in-service training, etc.

Cooperates and works together with all co-workers; plan and complete job duties with minimal supervisory direction, including appropriate judgment.

o Follows up as appropriate with supervisor, co-workers or consumers regarding reported complaints, problems and concerns. 

o Completes requirements for in-service training, acceptable attendance, uniform and dress codes 

including personal hygiene, and other work duties as assigned.

Provide quality nursing care to consumers in an environment which promotes their rights, dignity, freedom of choice, and their individuality as illustrated by the following: 

o Provide individualized attention, which encourages each consumer's ability to maintain or attain the highest practical physical, mental, and psychosocial well-being. 

o Receives training in, and carries out a variety of beginning level procedures and techniques in the 

care, treatment and/or habilitation of individuals with mental illness or developmental disabilities; 

documents care, treatment and progress in an individual's active treatment plan or other designated records.

o Assists professional therapists by performing beginning level activity program duties; leads individuals and participates in various educational, vocational, recreational and social activities; advises supervisor and professional therapists of individuals' behavior and reactions to programs; maintains 

activity supplies and equipment.

o Observes individuals' behavior and interacts with them in order to promote their rehabilitation and 

development; participates as a member of an individual habilitation team.

o Knowledgeable of the individualized care plan for consumers and provide support to the consumer

according to the care plan. Contribute to the care planning process by providing the charge nurse or 

other care planning staff with specific information and observations of the consumers' needs and 

preferences.

o Attend to the individual needs of consumers which may include assistance with grooming, bathing, 

oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, 

ambulation, and range of motion, communicating, or other needs in keeping with the individuals' care 

requirements. 

o Maintain the comfort, privacy, and dignity of each consumer in the delivery of services to them. 

Interact with consumers in a manner that displays warmth and promotes a caring environment. 

o Protect the personal belongings of each consumer including eyeglasses, dentures, hearing aids, 

furnishings, jewelry, clothing, memorabilia, etc. Promptly report missing items according to established 

policy and participate in efforts to locate missing items. 

o Observe consumers for changes in condition or behavior and promptly report these changes to 

appropriate licensed nursing personnel. 

o Practice careful, efficient, and no-wasteful use of supplies and linen and follow established charge 

procedures for consumer charge items. 

o Complete certified nursing assistant records documenting care provided or other information in 

keeping with department policies.

Requirements

Successful completion of state-approved Nursing Assistant Certification course or other education which 

meets state certification requirements. Must hold current Nursing Assistant Certification within current state of 

employment.

Successful completion of RSA Qualification training/exam.

Moderate reading, writing, grammar in the English language.

Uses tactful, appropriate communications in sensitive and emotional situations. 

Auditory and visual skills.

Must be able to work within and promote a team atmosphere.
