The Clinical Staff Pharmacist provides generalized pharmacotherapy services to adolescent, adult, and/or geriatric patient populations. The position prepares or supervises the preparation of routine pharmaceutical items and compounded sterile and non-sterile preparations. This position communicates and collaborates with physicians, Clinical Pharmacy Specialists and patients, to provide clinical consultation, monitor drug therapy, and offer cost-effective alternatives, as appropriate.

TWO SHIFTS PER WEEK. 8AM-430PM.
WHAT YOU CAN EXPECT

  • Consult with and advise other Healthcare professionals and patients on matters pertaining to selection, procurement, distribution, and safe and appropriate use of medications.

  • Maintain records, such as pharmacy files, inventories, controlled substance records, and registries.

  • Manage the disposition of controlled substances and other dangerous drugs in accordance with legal and regulatory requirements.

  • Monitor drug therapy regimen for contraindications, drug-drug interactions, drug-food interactions, allergies, appropriateness of drug and dose.

  • Provide clinical pharmacy services, as outlined in approved hospital policies, including but not limited to: pharmacokinetic consults, renal dosing, anticoagulation management, geriatric dosing management, route optimization, antimicrobial stewardship, and reviewing medications at transitions of care.

  • Participate in scheduled in-services, educational presentations, computer-based training, and staff meetings, to ensure ongoing understanding of and adherence to professional standards, regulatory requirements and organizational policies and practices.


QUALIFICATIONS

  • Required Bachelor's Degree Pharmacy

  • Preferred Doctorate Pharmacy

  • 1 year Hospital Pharmacy Preferred

  • Registered Pharmacist (RPh) - State Licensing Board
