Job description

Rochester Regional Health is seeking a Geriatric Physician to join our Eastern Region team, providing diverse Geriatric Care in our ElderONE (PACE) program, and Skilled Nursing Facilities (Clifton Springs Nursing Home & DeMay Living Center). This role also offers exciting teaching opportunities with fellows and medical students.

Position Highlights

  • Comprehensive Care: Deliver patient-centered care for older adults, focusing on preventive care, chronic disease management, and complex medical needs.
  • Teaching: Mentor Geriatric Fellows and medical students, contributing to their clinical development.
  • Collaborative Teamwork: Work with interdisciplinary teams, including ElderONE staff and SNF teams, for holistic care.
