Responsibilities

JOB RESPONSIBILITIES:

As a wellness and care management registered nurse, you will focus on patients through internal referrals from our clinicians to achieve health goals. The registered nurse while advocating for our patients in preventive visits, will also provide primary care nursing responsibilities.

Wellness and Care Management Nurse responsibilities & duties:

    Conduct initial assessments to understand patient's goals
  • Develop wellness plans in conjunction with patient
  • Offer wellness coaching and guidance
  • Motivate patients to reach their goals
  • Conduct group sessions and seminars on varies topics
  • Plan wellness events based on health and wellness monthly objectives
  • Provide coaching visits via in clinic, telehealth, and video
  • Care Management: Coordinating care with clinic providers and specialists and tracking patient treatment plans.

Primary Care Registered Nurse responsibilities & duties:

  • Deliver competent and skilled primary care nursing care to patients and families (including allergy injections, IV therapy, EKG, wound care) in accordance with established policies and procedures
  • Triages patient via phone or in clinic
  • Support emergency care with guidance from our clinicians
  • Supports clinic operations including medication refills, prior authorization, FMLA paperwork completion
  • Care coordination with patients and families
  • Establish long-term relationships with patients while providing care across the life span
  • Ensures accurate, concise, timely and complete documentation of results and paperwork
  • Ability to manage time, prioritize and multi-task in a busy environment
  • Ability to adapt to changing business needs such as scheduling and working additional/different hours/shifts when appropriate

Qualifications

JOB REQUIREMENTS:

Education, License, and Certifications:

  • Bachelor's Degree in Nursing from an accredited university
  • Current license to practice professional nursing in the State of Texas required
  • Certification in health coaching program or ability to secure preferred
  • BLS certification approved by the American Heart Association required
  • Bachelor's Degree in Nursing from an accredited college or university or Graduate of an accredited Registered nursing program
  • Current registration/license in the state where employed
  • Previous occupational health experience is highly preferred; COHN is requested
  • Customarily at least one year of demonstrated experience in occupational health nursing or related field

  • Proficient in electronic health record systems
  • Ability to make decisions or solve problems by using logic to identify key facts, explore alternatives, and propose quality solutions

  • The ability to properly handle sensitive and confidential information (including HIPAA and PHI) in accordance with federal and state laws and company policies
  • Demonstrated excellent communication and motivational skills
  • Demonstrated willingness to participate in Continuing Education Units (CEU) in nursing to maintain certification and state license
