POSITION SUMMARY – RN Case Manager POSITION DUTIES – Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient’s medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning. MINIMUM REQUIRED QUALIFICATIONS –
  • TX/compact RN license
  • BLS
  • 2 years case management experience in an acute hospital setting
  • Acute hospital experience during the past year with no more than a 90-day break
LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – 8a-5p SYSTEMS – Meditech/Meditech Expanse START DATE – ASAP
