Job Title: Registered Nurse Case Manager

Description:
Conduct initial patient and family assessments for every inpatient.
Determine psycho-social needs and discharge plans.
Complete care coordination assessments within the first 48 hours of admission.
Document patient data, defined problems, assessment goals, treatment plans, and follow-up activities on a weekly basis in medical charts.
Participate in multidisciplinary rounds as a team member.
Provide consultative support to staff regarding patients and families.
Communicate clinical status updates to patients, families, and other involved individuals.
Regularly evaluate treatment plans in relation to patient progress toward goals.
Review final discharge plans and instructions with patients, families, or responsible parties.
Document discharge dates and any services patients were referred to in medical records.
Coordinate discharge plans with the entire rehabilitation team.
