Bonuses and Benefits

MVHS is offering a sign-on bonus to new and experienced RN Case Managers up to $10,000!

This bonus will be paid over the course of two years working at MVHS and will scale based on experience and employee status when hired.

Other benefits include: Health benefits starting on the first of the month after hire date, 401(k) plus match, Health Spending Accounts, PTO, and tuition reimbursement.

Pay Range: $40- $56 per hour.

Job Summary

Our Case Management team is expanding!

We are seeking RN Case Managers to join our team. Those with a background in Case Management or looking to explore a new opportunity in nursing are encouraged to apply!

As an RN Case Manager, you will use your clinical expertise to assess patients across the continuum of care, coordinate individualized care plans, and collaborate closely with physicians and multidisciplinary teams. You will advocate for patients, review diagnostic results and level-of-care-needs, and help ensure patients receive the right care at the right time. This is a great opportunity for an RN who enjoys clinical decision-making, care coordination, and making a meaningful impact on patient outcomes

Work schedule:

Hours are assigned based on clinical and operational needs, as well as staff availability, and may vary. Typical shifts may include: Monday-Friday, 8:00am-4:00pm with occasional weekend rotation.

Core Job Responsibilities
  • Coordinates discharge planning to assure that the patient progresses through the continuum of care and is discharged to the least restrictive environment.
  • Coordinate the hospital activities concerned with case management and discharge planning.
  • Ensure compliance with quality patient care and regulatory compliance.
Education/Experience Requirements

Required: 

  • Minimum of two (2) years of nursing experience.
  • Graduate of an accredited nursing program. Licensed/registered RN in NY State.

Preferred: 

  • Bachelor Nursing degree is preferred.
  • Experience in a hospital Case Management/discharge planning
Licensure/Certification Requirements

Required: 

  • Maintain current professional licensure in nursing or professional filed of certification.

Preferred: 

  • Appropriate certification in the case management preferred (e.g. Commission for Case Management Certification (CCMC) or Association of Rehabilitation Nurses).
