RN Care Manager
Rivertown Health & Rehabilitation Center
Montgomery, AL

Rivertown Health & Rehabilitation Center is seeking a compassionate and experienced Registered Nurse (RN) Care Manager to join our clinical leadership team. This is an excellent opportunity for an RN with experience in skilled nursing, rehabilitation, case management, admissions, MDS, or care coordination who is passionate about improving resident outcomes and leading interdisciplinary care.

As our RN Care Manager, you will coordinate resident care from admission through discharge, collaborate with physicians and the interdisciplinary team, support quality initiatives, and help ensure exceptional care for our short term rehabilitation and long term care residents.

Responsibilities

  • Coordinate resident care throughout the stay, from admission to discharge.

  • Collaborate with physicians, nursing, therapy, admissions, and social services.

  • Lead interdisciplinary care plan meetings.

  • Assist with discharge planning and transition of care.

  • Coordinate care for Medicare, managed care, and long term care residents.

  • Communicate with residents and families regarding care plans and progress.

  • Ensure accurate clinical documentation and regulatory compliance.

  • Support quality improvement efforts and reduce avoidable hospital readmissions.

Qualifications

  • Current Alabama Registered Nurse (RN) license required.

  • Experience in skilled nursing, rehabilitation, long term care, case management, MDS, admissions, or care coordination preferred.

  • Strong clinical assessment, communication, and organizational skills.

  • Ability to prioritize multiple responsibilities while working collaboratively with an interdisciplinary team.
