This Federally Qualified Health Center (FQHC) delivers comprehensive primary care services to patients of all ages. The facility operates with established clinical workflows to support consistent and quality care delivery within the community health setting. Clinicians on assignment will work within a structured outpatient environment designed to serve the primary care needs of the surrounding community.

About the Facility Location

Medford, Oregon, situated in the Rogue Valley, offers a range of outdoor and leisure attractions suited to varied interests. The area provides access to natural destinations such as Upper and Lower Table Rock, Roxy Ann Peak, Prescott Park, and the Natural Bridge, as well as whitewater rafting on the Rogue River and recreational opportunities at Diamond Lake Resort. Local landmarks including Medford Railroad Park and Rogue Valley Mall are also available to clinicians on assignment throughout the region.

About the Clinician's Workday

The client is ideally seeking a Nurse Practitioner or PA who is currently enrolled in Medicare and has at least 2 years of primary care experience. The schedule is Monday through Friday between 7:30 a.m. and 6:30 p.m., with consideration for four 10-hour shifts. There is no call required, and providers will see approximately 10-15 patients per day. The patient population includes all ages and consists primarily of routine primary care visits, gender-affirming care, substance abuse treatment, mental health concerns, physicals, and sick visits. Candidates must have an active Oregon license, Oregon DEA, and BLS or ACLS certification to be presented. Experience in urgent care is not preferred, as it does not align well with the primary care focus of this clinic. Preferred but not required experience includes gender-affirming care and substance use disorder treatment. Providers will work alongside a collaborative team of family medicine physicians, nurse practitioners, and behavioral health specialists, utilizing the EPIC EMR system. COVID and flu vaccinations are preferred but not required.
