Job description

Are you passionate about helping older adults and making a meaningful difference in your community? The Senior Hub-Meals on Wheels of Fairfield County is seeking a full-time In-Home Services Manager to oversee our Homemaking and other in-home service programs that help older adults remain safe, independent, and connected in their homes.

The ideal candidate is organized, compassionate, and detail-oriented, with a strong ability to work effectively with both clients and staff in a fast-paced environment.

ESSENTIAL JOB FUNCTIONS
  • Supervise the Homemaker, Emergency Response, and Adaptive equipment programs and staff
  • Oversee and maintain the In-Home Repair Program
  • Conduct initial visits and develop care plans for the Homemaker program
  • Coordinate contracted providers and programming for the In-Home Services Department
  • Coordinate and schedule in-home services with contracted or internal staff, to include but not limited to, personal care, homemaker services, respite care, and home repairs
  • Work effectively with community providers to ensure in-home service referrals are fulfilled
  • Conduct monthly reconciliation of service units and provider billing
  • Develop an annual continuing education curriculum for In-Home Services staff
  • Maintain training records as required by COAAA
REQUIREMENTS

Candidates must meet one of the following qualifications:

  • Registered Nurse (RN)
  • Licensed Practical Nurse (LPN) working under an RN
  • Bachelorโ€™s degree in Human Services
  • State Tested Nursing Assistant (STNA) with at least 2 years of experience

Additional Requirements:

  • Supervisory experience required
  • Valid driverโ€™s license and good driving record required
