Shepherd Hospice LLC is seeking a Social Worker to join our team and support patients and families through compassionate, person-centered care in New Albany, MS and surrounding areas.
Position SummaryAs a Social Worker, you will provide professional psychosocial support to patients and their support systems, helping navigate the emotional, practical, and community resources associated with serious illness and end-of-life care. You will work collaboratively with an interdisciplinary team to assess needs, develop care plans, and ensure services are coordinated effectively.
This is a full-time remote role based in New Albany, MS, with opportunities to engage with patients and families through ongoing case management, advocacy, and supportive counseling.
Key ResponsibilitiesConduct psychosocial assessments to identify needs, strengths, risks, and barriers to care, and recommend appropriate interventions.
Develop, implement, and update care plans in collaboration with the interdisciplinary team to support patient goals and family needs.
Provide counseling, crisis support, and education to help patients and families cope with stressors, changes in health, and care transitions.
Coordinate referrals and connect patients with community resources, financial assistance options, and supportive services.
Maintain accurate, timely documentation and ensure compliance with applicable care standards and organizational policies.
Bachelor’s degree in Social Work or a closely related field, with relevant experience in a clinical or community-based setting.
Demonstrated ability to conduct assessments, engage with individuals and families, and provide supportive counseling.
Strong knowledge of ethical practice, confidentiality, and professional boundaries in social work.
Excellent communication skills, including the ability to build trust with patients, families, and care teams.
Ability to manage priorities, handle sensitive situations with professionalism, and maintain accurate records.
Experience in hospice, palliative care, or case management environments.
Experience working with diverse patient populations and adapting communication to meet individual needs.
Familiarity with care coordination processes and interdisciplinary team collaboration.

Annual salary: $40,000.00 - $72,000.00
