In this full-time role, the LCSW will be responsible for leading psychoeducational and process-oriented therapy groups, conducting individual therapy sessions, performing clinical assessments, implementing effective interventions, and managing various case management responsibilities.

  • Confirm discharge appointments with patients and their loved ones ahead of time, ensuring all parties are prepared for the upcoming changes.
  • Communicate with referral sources and outpatient providers at key points—upon admission, mid-treatment, and pre-discharge—to guarantee continuity of care and seamless transitions for patients.
  • Facilitate patient transfers to higher levels of care when necessary, ensuring that each patient receives the appropriate level of support.
