• Conduct in-depth clinical assessments to evaluate patients' mental health needs, developing individualized treatment plans that incorporate evidence-based practices.

• Utilize solution-focused therapeutic interventions, adapting strategies to address unique patient circumstances and challenges in a hospice context.

• Document clinical activities by maintaining accurate and timely records, ensuring compliance with all regulatory standards while minimizing administrative burden.

• Collaborate with a multidisciplinary team of healthcare professionals, including doctors, nurses, and chaplains, to provide holistic care tailored to the specific needs of patients and their families.

• Provide education and support not only to patients but also to their families, offering resources and guidance to manage grief and improve coping mechanisms through the hospice process.

• Stay informed on the latest research and developments in mental health practices, integrating new methodologies into care to continuously enhance the effectiveness of therapeutic interventions.

The LCSW will work primarily with patients facing life-limiting illnesses and their families, addressing a range of psychological, emotional, and social concerns. This role is crucial in providing support during a vulnerable time, enhancing the patients’ quality of life and assisting their loved ones in navigating the complexities of end-of-life care.

• A Master’s degree in Social Work from an accredited institution.

• A valid LCSW license issued by the state of Texas.

• Demonstrated knowledge of solution-based psychotherapy modalities and comfort working within a hospice or palliative care environment.

• Exceptional communication skills, coupled with the ability to problem-solve in high-pressure situations.

• Experience working in hospice, palliative care, or other end-of-life settings.

• A background in grief counseling and supporting families through bereavement.

• Familiarity with telehealth platforms and virtual patient engagement tools.

• Annual salary ranging from USD 80,000 to 100,000, commensurate with experience and credentials.
