• Conduct thorough mental health assessments, taking into consideration personal histories, current challenges, and potential risk factors to develop tailored treatment plans.
• Facilitate individual and family counseling sessions utilizing evidence-based therapeutic techniques, such as cognitive-behavioral therapy (CBT) and trauma-informed approaches, to address emotional distress and enhance coping skills.
• Conduct crisis intervention initiatives, ensuring rapid and effective response to urgent mental health issues as they arise.
• Collaborate with a diverse team of clinicians to ensure comprehensive treatment approaches, integrating various professional insights for holistic care.
• Monitor patient progress through ongoing evaluations, adjusting treatment methodologies as necessary to achieve optimal outcomes.
• Provide education and resources to patients and families to foster understanding of mental health issues, thereby promoting better health choices.

Our primary focus is on patients within a hospice setting, including terminally ill individuals and their families. You will be instrumental in providing emotional support during profound transitions, helping patients navigate grief, loss, and psychological distress. • Education: A Master’s degree in Social Work from an accredited institution.
• Licensure: Valid LCSW licensure in the state of Florida.
• Experience: At least two years of post-licensure experience, with a preferred focus on mental health or hospice social work.
• Deep understanding of mental health disorders, treatment modalities, and crisis intervention techniques.
• Proficiency in conducting comprehensive assessments and formulating effective treatment plans based on individual patient needs.
• Strong interpersonal skills and the ability to communicate effectively with patients from diverse backgrounds.

• Experience working in hospice or end-of-life settings is highly desirable.
• Demonstrated competency in delivering group therapy and educational sessions to promote community health.
• Familiarity with electronic health record (EHR) systems to ensure proper documentation and patient follow-up.
