As a remote worker at Heart Mental & Wellness Clinic, you will collaborate with a diverse team of clinicians and support staff who prioritize a culture of sensitivity and mutual respect. • Act as a critical liaison between clients, families, and outpatient providers, ensuring open lines of communication and effective care coordination.
• Engage with insurance companies to manage pre-certifications and utilization reviews, advocating for client needs.
• Participate actively in discharge planning and aftercare strategies, facilitating smooth transitions for clients returning home or moving on to alternative care settings.
• Offer therapeutic meal support, guiding clients in establishing a healthy relationship with food through empathetic and supportive interactions.
• Deliver safe, compassionate, and affirming care tailored to each client's unique situation, taking into account their diverse identities and backgrounds.

Your client base will primarily encompass older adults dealing with complex mental health challenges, including grief, anxiety, depression, and end-of-life concerns. By providing specialized support in these areas, you will play an integral role in enhancing the quality of life for individuals at a critical juncture, contributing to their mental and emotional wellness.

• A Master’s degree in Social Work from an accredited institution is required.
• Possession of a valid LCSW license in the State of New York is mandatory.
• Experience working with older adult populations is highly preferred, highlighting your capability to address their specific needs and challenges.
• Strong clinical and diagnostic skills to assess and deliver effective interventions.
• Excellent communication and interpersonal skills, enabling effective collaboration with team members, clients, and external partners.
• Ability to work both independently and within a team-oriented environment, demonstrating flexibility and initiative.

While a minimum of one year of relevant experience is required, additional experience in palliative care, hospice, or related fields would enhance your candidacy. Familiarity with therapeutic methodologies and engagement strategies tailored for older adults will be advantageous.

The position offers a competitive annual salary ranging from $80,000 to $100,000, commensurate with experience and qualifications.
