Key Responsibilities
  • Conduct thorough assessments to determine the counseling needs of clients, focusing specifically on co-occurring disorders.
  • Utilize the DSM (Diagnostic and Statistical Manual) criteria to diagnose substance use disorders and evaluate clients for appropriate levels of care based on ASAM (American Society of Addiction Medicine) standards.
  • Develop tailored client treatment plans that include substance abuse counseling components and regularly update them to reflect client progress.
  • Create discharge and continuum of care plans that facilitate client transitions to the appropriate levels of ongoing care post-treatment.
  • Maintain accurate and up-to-date documentation of all counseling sessions, assessments, and follow-ups in the client’s clinical records.
  • Manage a client caseload while ensuring that all case management duties, including utilization reviews, are met effectively.
  • Coordinate support services for clients and their families, as well as document all services provided in clinical records.
  • Communicate and collaborate with referral sources, such as legal, educational, and employment agencies, to ensure holistic care for clients.
  • Facilitate crisis interventions by utilizing a multidisciplinary team approach, addressing immediate needs effectively.
Patient Population / Client Focus

Your primary focus will be on clients struggling with addiction and co-occurring mental health issues. You will be tasked with providing empathetic and compassionate counseling that respects the individuality and unique backgrounds of each client. Adopting a person-centered approach, you will work towards empowering clients, helping them navigate their recovery paths successfully.

Qualifications
  • A Master's degree in Social Work from an accredited institution.
  • Active Licensed Clinical Social Worker (LCSW) certification.
  • Demonstrated knowledge of substance use disorder treatment modalities and mental health counseling methods.
  • Exceptional interpersonal skills with a strong emphasis on cultural competency and the ability to establish therapeutic boundaries.
  • Proficient in using electronic health record (EHR) systems for documentation and case management.

Preferred Experience
