West Valley Psychiatric Services is seeking a dedicated and experienced Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote capacity. This role focuses on providing comprehensive counseling services to individuals and families, specifically within our hospice center setting. As part of our hospice center, you will collaborate with experienced multi-disciplinary teams, including medical professionals and recovery specialists, to deliver holistic care that addresses both mental health and substance use disorders.

• Assess the counseling needs of clients through thorough evaluations and engage in ongoing assessments to tailor treatment plans effectively.
• Utilize DSM (Diagnostic and Statistical Manual) criteria to diagnose substance abuse disorders and determine clients’ levels of care based on ASAM (American Society of Addiction Medicine) criteria.
• Develop comprehensive treatment plans that incorporate substance abuse counseling, integrating client input to ensure the plans are meaningful and actionable.
• Collaboratively prepare discharge and continuum of care plans to facilitate smooth transitions to appropriate levels of care, helping clients maintain their recovery.
• Document all counseling, assessments, referrals, and follow-ups meticulously in clients’ clinical records, ensuring all legal and ethical standards are upheld.
• Manage a caseload effectively, performing regular case management and utilization review updates as necessary to maintain high standards of care.
• Facilitate coordination of support services for clients and their families, ensuring comprehensive care and tracking all provided services in client records.
• Engage with external referral sources, including legal, educational, and employment-related entities, to build a network of support for clients.
• Respond to crisis situations utilizing a multi-disciplinary approach, implementing interventions that prioritize client safety and wellness.

In this role, you will primarily serve clients facing co-occurring disorders in a hospice center, which requires a nuanced understanding of both mental health issues and substance use disorders. You will be instrumental in guiding clients through their recovery journeys, providing therapeutic support and intervention during what can be one of the most challenging times of their lives.

• Must possess a valid LCSW license in Colorado and have a Bachelor's degree in Social Work or a related field.
• Demonstrated professional integrity, exemplary therapeutic boundaries, and cultural competency in working with diverse populations.
• Strong self-motivation, self-sufficiency, and organizational skills to manage multiple client needs effectively.
• Excellent communication skills to engage with clients, families, and multidisciplinary teams.

• Experience in co-occurring counseling, with a focus on substance use disorders and mental health.
• Familiarity with treatment planning methodologies and best practices in hospice or end-of-life care settings.
• Proficiency in utilizing electronic health records for documentation and collaboration with other care providers.

This position offers a competitive salary ranging from USD 70,000 to 100,000 annually, commensurate with experience and qualifications. We provide an extensive benefits package that includes:
