Key Responsibilities

As a Licensed Clinical Social Worker at Crossroad Behavioral Health Services, your primary responsibilities will include:

  • Conducting thorough assessments of client counseling needs, utilizing applied methodologies and assessment tools.
  • Evaluating clients for substance use disorders using the DSM (Diagnostic and Statistical Manual) criteria and determining the appropriate level of care based on ASAM (American Society of Addiction Medicine) criteria.
  • Developing and preparing individualized treatment plans that encompass the substance abuse counseling portion and align with client goals.
  • Creating comprehensive discharge and continuum of care plans to facilitate successful transitions for clients moving through different levels of care.
  • Documenting all interactions, assessments, treatment plans, and follow-ups in clientsโ€™ clinical records, ensuring compliance with ethical standards and regulatory requirements.
  • Maintaining an active client caseload, overseeing their case management needs, and providing necessary updates related to utilization reviews.
  • Coordinating support services for clients and their families, documenting all services provided in the clientโ€™s record.
  • Engaging with referral sources such as legal entities, schools, and employers to ensure comprehensive care.
  • Facilitating interventions for crisis-oriented issues, leveraging a multi-disciplinary team approach to address the unique needs of your clients.
Patient Population / Client Focus

Your work will primarily involve individuals dealing with a range of mental health issues and substance use disorders. By tailoring your therapeutic approaches to fit the needs of each client, you will empower them on their journeys toward recovery and self-sufficiency. Qualifications
