• Perform comprehensive clinical assessments to understand the unique needs of patients and develop individualized treatment plans that incorporate evidence-based therapeutic interventions.

• Deliver tailored therapeutic interventions, utilizing solution-based practices that promote patient engagement and empower families through education and support.

• Maintain meticulous clinical records and documentation, ensuring adherence to regulatory standards and maintaining patient confidentiality.

• Collaborate closely with a multidisciplinary team, including physicians, nurses, and other healthcare professionals, to provide coordinated and holistic care.

• Participate in case consultations and peer review meetings to discuss treatment plans, challenges, and share best practices, contributing to the overall enhancement of care quality.

As an LCSW within our hospice center, you will work with a diverse patient population, including individuals facing life-limiting illnesses and their families. Your role is pivotal in helping patients navigate their emotional and psychological challenges, providing them and their families with the necessary tools and support to cope with the complexities of end-of-life care.

• A Master’s degree in Social Work from an accredited institution is required.

• A valid Licensed Clinical Social Worker (LCSW) license in the state is mandatory.

• A minimum of three years of clinical experience, with a focus on psychotherapy and end-of-life care, is essential.

• Demonstrated proficiency in solution-focused therapy and evidence-based practices.

• Exceptional communication and interpersonal skills, with a strong ability to relate to patients and families in a compassionate and empathetic manner.

• Experience working in hospice, palliative care, or similar settings is highly desirable.

• Familiarity with electronic health record (EHR) systems and telehealth platforms.

• Previous involvement in case management and interdisciplinary collaboration.

The compensation range for this position is between USD 80,000 and 100,000 annually, commensurate with experience and qualifications.
