As the Referral and Admission Coordinator, you will be responsible for managing the referral process for assigned agencies. The Referral and Admission Coordinator works closely with other members of the team to ensure adequate coordination and transition for patients with an order for home health services. The Referral and Admission Coordinator is responsible for processing all incoming referrals through e-portals and other methods and may have additional duties assigned. This role is primarily a remote position.

Primary Responsibilities:

  • Manage incoming referrals through e-portals and other referral sources for assigned home health agencies
  • Gather demographic and clinical information from clients, physician offices, and referral sources to complete referrals
  • Communicate patient and referral information with staff, referral partners, physicians, facilities, patients, and families
  • Provide general information about agency services and triage incoming calls and messages from referral sources and partners
  • Serve as a liaison between clinical staff, referral partners, and business development teams to support a seamless experience
  • Maintain accurate and timely documentation in the electronic medical record system, including tracking non-admitted patients
  • Comply with state and federal referral and intake requirements; licensed LPNs, LVNs, or RNs may perform additional clinical duties as needed

Required Qualifications:

  • Graduate of an accredited LPN, LVN or RN school. License, Registration, and / or Certification in the state of practice
  • 1 years of healthcare experience
  • Sales support or customer service experience
  • Solid communication skills and the ability to work both with a team and independently
  • Computer experience including knowledge of Microsoft Office
  • Proven excellent critical thinking and problem-solving skills

Preferred Qualification:  

  • Bachelor's degree

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $14.00 to $26.20 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
