As an LMSW working with Gotham Enterprises Ltd., your responsibilities will encompass the following key areas:

• Conduct comprehensive clinical assessments of patients' emotional and social needs, crafting tailored treatment plans based on established best practices.
• Facilitate a variety of therapeutic interventions, such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT), adapting approaches according to individual patient needs and preferences.
• Create and maintain thorough, accurate clinical documentation to ensure compliance with HIPAA regulations and uphold the highest standards of patient confidentiality.
• Engage collaboratively with the interdisciplinary treatment team, including psychiatrists and psychologists, to deliver coordinated and patient-centered care that effectively addresses the multifaceted needs of each patient.
• Educate patients and families about mental health challenges, treatment modalities, and available community resources that may enhance their care journey.
• Regularly participate in case reviews and peer consultation meetings to share insights, seek feedback, and enhance treatment strategies within the clinical framework.
• Stay informed on the latest research and innovations in social work and mental health to integrate contemporary evidence into your clinical practice.
• Manage a diverse caseload, ensuring that all treatment objectives are met timely and professional standards are upheld, as dictated by the National Association of Social Workers (NASW).

Your role will primarily focus on providing mental health support to patients in a hospice care setting, many of whom are dealing with terminal illnesses and complex emotional reactions associated with end-of-life care. You will offer empathetic support to families adjusting to significant life changes, ensuring they are equipped with the tools to cope with their circumstances.

• A Master’s Degree in Social Work from an accredited institution is required.
• A valid license to practice as a Licensed Master Social Worker (LMSW) in New York State.
• A minimum of three years of clinical experience providing psychotherapy services, with a background in working with diverse patient populations.
• Proficiency in evidence-based intervention techniques and a solid understanding of solution-focused therapy.
• Exceptional interpersonal an communication skills that facilitate strong therapeutic relationships.
• Demonstrable problem-solving capabilities and critical thinking skills.
• The ability to collaborate effectively within a multidisciplinary team to promote coordinated patient care.

• Experience specifically in hospice or palliative care settings is highly desirable.
• Familiarity with telehealth practices, as remote interactions may form part of the care delivery process.
• Knowledge of community resources and referral networks relevant to patients facing chronic and terminal illnesses.
