Key Responsibilities
  • Conduct comprehensive assessments and evaluations to determine the mental health needs of patients, developing personalized treatment plans to guide their recovery.

  • Organize and coordinate home visits when necessary, ensuring that observations, data collection, and chart reviews are performed accurately and efficiently.
  • Participate in Interdisciplinary Team Meetings to discuss cases, share best practices, and drive continuous improvements in service delivery.
  • Record detailed monthly and quarterly progress notes to track patient development, adjust treatment strategies, and fulfill compliance requirements.
  • Provide ongoing support through follow-ups, home visits, and observations, ensuring patients feel supported in their journey to mental wellness.
  • Train and educate team members on the implementation of program plans and proper documentation practices, fostering a culture of competency and adherence to procedures.
  • Act as a liaison with the community and mental health organizations to facilitate outreach, collaboration, and resource sharing.
Patient Population / Client Focus

The Licensed Clinical Professional Counselor will serve a diverse clientele, including both adults and youth facing a variety of mental health challenges such as anxiety, depression, trauma, and substance use disorders. Qualifications
