As an LCSW with our hospice center, you will be part of a multidisciplinary team collaborating to address the emotional and psychological needs of patients facing terminal illnesses. • Conduct thorough clinical assessments for hospice patients to develop tailored treatment plans.
• Provide evidence-based therapeutic interventions, including individual and family counseling, to address complex emotional and psychological challenges.
• Maintain comprehensive and timely clinical documentation to ensure compliance and effective patient care.
• Work collaboratively with a diverse multidisciplinary team, including nurses, physicians, and other social workers, to optimize patient outcomes.
• Deliver education, support, and resources to patients and their families, fostering understanding of the hospice process and available options.
• Stay abreast of the latest therapeutic techniques and mental health advancements, incorporating new knowledge into practice.

In your role as an LCSW, you will have the privilege of working with a diverse patient population facing various challenges related to terminal illnesses. This includes providing emotional and psychological support to patients and families as they navigate grief, loss, and end-of-life issues. Your insights and expertise will help enhance their coping mechanisms, support their emotional well-being, and improve their overall quality of life.

• A Master’s degree in Social Work from an accredited institution.
• A valid and active LCSW license in the state of Georgia.
• Strong knowledge of solution-focused and evidence-based therapeutic interventions.
• Exceptional communication skills, with a knack for building rapport and trust with patients and families.

• A minimum of 2 years of experience in a clinical setting, with a focus on hospice, palliative care, or mental health.
• Experience working in a multidisciplinary team environment.
• Familiarity with electronic health record systems and data entry practices.
