Key Responsibilities

  • Provide compassionate support and guidance to patients and families, assisting them in coping with various environmental and psychological challenges associated with illness.
  • Independently manage a caseload that varies in clinical acuity, which includes identifying patients with potential psychosocial needs through proactive case finding.
  • Engage in team conferences and actively participate in interdisciplinary rounds to promote comprehensive patient care.
  • Offer therapeutic services including supportive counseling, grief counseling, and crisis intervention to empower patients and families dealing with illness and transitioning needs.
  • Deliver psychoeducation on treatment adaptations related to diagnosis and illness, including assisting youth in transitioning to adult services.
  • Contribute to quality assurance and improvement initiatives related to social work services, aiding in the development of relevant policies and guidelines.
  • Conduct mental health diagnoses and participate in treatment planning and execution when clinically indicated.
  • Supervise and mentor MSWs and student interns, ensuring a nurturing environment for professional growth.
  • Maintain accurate electronic medical records and documentation in compliance with departmental policies, providing comprehensive verbal and written updates as needed.
Patient Population / Client Focus
