As a Licensed Clinical Social Worker, your day-to-day responsibilities will be diverse and impactful. You will be tasked with:

• Conducting comprehensive clinical assessments, identifying patients' mental health needs, and developing personalized treatment plans that embrace solution-focused methodologies.

• Implementing evidence-based therapeutic interventions tailored to individual patient goals, ranging from crisis intervention and supportive counseling to complex case management.

• Thoroughly documenting clinical records in compliance with facility policies and state regulations, ensuring accuracy and timeliness in all documentation.

• Collaborating with a multidisciplinary treatment team, which may include psychologists, physicians, and nurses, to provide holistic care and support to patients and their families.

• Offering education and support to patients and their families to enhance their understanding of mental health conditions and treatment options.

• Staying abreast of the latest research and developments in the field of mental health to continuously improve your clinical practice and contribute to the organization’s best practices.

The primary focus of this role is to work with a diverse population of patients, including adults dealing with emotional distress, trauma, chronic illness, and end-of-life issues. As an LCSW in our hospice setting, you will provide compassionate care that addresses not just the psychological aspects of their conditions but also supports patients and families through the accompanying emotional journeys.

The ideal candidate for this position will possess:

• A Master’s degree in Social Work from an accredited institution.

• A valid and active LCSW license in the state of Texas.

• Strong knowledge of solution-focused therapy interventions, with experience in applying these methodologies in clinical practice.

• Exceptional communication skills, both verbal and written, with the ability to establish rapport with patients and effectively collaborate with interdisciplinary teams.

While the essential qualifications are a must, we highly prefer candidates with experience in hospice or palliative care settings, as well as familiarity with issues related to chronic illness and grief management. Previous engagement in trauma-informed care practices will also be advantageous in delivering the highest standard of patient support.
