In your capacity as a Licensed Clinical Social Worker, you will undertake a variety of impactful responsibilities, including but not limited to:

• Conducting thorough assessments and utilizing appropriate diagnostic tools, such as the Diagnostic and Statistical Manual of Mental Disorders, to inform individualized care plans.
• Monitoring patients' progress, adjusting treatment strategies, and applying evidence-based therapeutic modalities tailored to their mental health needs.
• Documenting clear and informative treatment records and progress notes within regulatory guidelines and using modern electronic health record systems.
• Collaborating with patients, their families, and multidisciplinary teams to create comprehensive treatment strategies that respect cultural diversity and address special needs.
• Coordinating referrals among various healthcare providers to ensure smooth transitions in care and comprehensive treatment pathways for patients.
• Providing mentorship and educational opportunities for interns, students, and volunteers, fostering a culture of learning and growth within the team.

To be considered for this position, candidates must possess the following:

• A Master’s degree in Social Work from an accredited institution.

• Prior experience in hospice care or outpatient therapy settings.

The position offers a competitive annual salary ranging from $90,000 to $105,000, commensurate with experience and expertise. Additional benefits include:
