Key Responsibilities

• Provide individualized counseling and educational support to patients and their families, targeting the impacts of mental health on their overall well-being.
• Advocate for patients by ensuring their rights are upheld and needs are met through the navigation of protective services.
• Exhibit a strong awareness of ethical decision-making processes while supporting patients and their families through advocacy.
• Ensure compliance with legal reporting requirements concerning vulnerability or suspected abuse situations.
• Engage in care system processes that prevent unnecessary readmissions by identifying at-risk patients and developing proactive care strategies.
• Participate in transition conferences to effectively plan for patient discharges and reintegration into the community.
• Utilize advanced electronic medical record systems to document patient progress, treatment plans, and interventions aimed at improving mental health outcomes.
• Collaborate with an interdisciplinary team to anticipate patient needs and overcome barriers in care delivery.
• Maintain comprehensive knowledge of government and private payer networks to facilitate smooth transitions for patients.
• Support patients in connecting with community resources to address financial, social, and emotional needs throughout their care journey.

Patient Population / Client Focus

In this role, you will work with a diverse population of patients experiencing a range of mental health issues influenced by acute, chronic, or terminal illnesses. This includes providing support for their families, understanding the emotional toll of these conditions, and implementing therapeutic approaches that resonate with their unique circumstances. Your efforts will contribute significantly to enhancing quality of life for both patients and their caregiving networks.

Qualifications

• A Master’s or Doctorate degree in clinical psychology, counseling, or clinical social work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD) from an accredited educational institution.
• Possession of an independent license in the state where you reside (LCSW, LMFT, LPC) that is recognized in the United States.
• Strong electronic communication skills, particularly in managing electronic mail and telehealth systems.
• Ability to manage a significant caseload diligently while interfacing effectively with large healthcare providers and administrative support staff.
• A passion for making a positive impact on the lives of individuals and families facing behavioral health challenges.

Preferred Experience

• Experience working in outpatient mental health settings.
• Familiarity with electronic medical record systems and optimization of documentation processes.
• Demonstrated capability in interdisciplinary collaboration, working effectively among various healthcare professionals.
