Key Responsibilities

In your role as a Remote Licensed Clinical Social Worker, you will:

  • Conduct comprehensive assessments of seniors’ psychosocial needs, taking into consideration their mental, emotional, and social challenges.
  • Develop individualized care plans in collaboration with clients and their families, aligning with each client’s specific goals and preferences.
  • Provide counseling and emotional support to clients and their families to navigate transitions and cope with challenges unique to aging.
  • Advocate for clients, ensuring their rights are upheld and that they have access to vital community resources and services.
  • Coordinate with an interdisciplinary team, including healthcare professionals and caregivers, to provide holistic care.
  • Conduct regular welfare checks to monitor client safety, well-being, and living situations.

  • Document all client interactions, assessments, and progress accurately to ensure accountability and compliance with ethical standards.
Patient Population / Client Focus

This role is centered on serving seniors, who often face multiple, complex challenges including chronic illness, mental health issues, and life transitions such as loss of independence or loneliness. Your work will directly contribute to improving their quality of life by addressing these challenges proactively.

Qualifications
  • A Master’s degree in Social Work (MSW) from an accredited institution.
  • Current, valid LCSW-C licensure in the state of Maryland.
  • Demonstrated experience of a minimum of two years in a social work practice, specifically with a focus on geriatric care.
  • Superior knowledge of social work principles, including those pertinent to aging populations.
  • Exceptional skills in assessment, problem-solving, and communication, particularly in a remote capacity.
  • Ability to build rapport with seniors and families, demonstrating empathy and cultural competence.
Preferred Experience

While the minimum requirement is two years of experience, we prefer candidates who have worked specifically in community centers or similar environments that focus on geriatric populations.
