• Conduct comprehensive clinical assessments, gathering detailed insights and histories to inform treatment plans tailored to the unique needs of each patient.
• Develop, implement, and adjust evidence-based treatment plans utilizing solution-focused therapy and other therapeutic interventions suited to various mental health conditions.
• Maintain current and accurate clinical records, ensuring compliance with state and federal regulations, patient confidentiality standards, and organizational protocols.
• Actively collaborate with our multidisciplinary team, participating in regular case consultations and interdisciplinary meetings to ensure coordinated and holistic care for our clients.
• Provide education and guidance to both patients and their families about mental health challenges, fostering an understanding that empowers them in their treatment journey.
• Stay informed on the latest research and developments in mental health care to continuously improve therapeutic outcomes and enhance service delivery.

In this role, you will work with a diverse group of clients, including individuals experiencing severe mental health conditions, chronic illnesses, and emotional crises. Our focus on hospice care emphasizes empathy and understanding, as many patients may deal with complex grief, end-of-life issues, or new diagnoses. As a social worker, you will help clients navigate these challenges and access necessary resources, ensuring they receive the care and support they need.

• A Master’s degree in Social Work from an accredited institution.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of Kansas.
• At least three years of clinical experience, specifically focusing on psychotherapy and mental health treatment.
• Demonstrated proficiency in solution-based therapeutic techniques and evidence-based practices.
• Strong communication and interpersonal skills that facilitate effective collaboration with team members and clients.
