Key Responsibilities

As an LCPC, you will:

  • Assist primary care providers in identifying and managing mental health and psychosocial challenges faced by patients, serving as a vital member of the multidisciplinary care team.
  • Conduct in-depth patient intakes to perform diagnostic and functional evaluations, offering recommendations for treatment plans tailored to individual patient needs.
  • Provide brief yet impactful mental health interventions, ensuring patients receive the timely support they require.
  • Advise primary care providers on appropriate patient referrals to specialty mental health facilities when necessary.
  • Initiate follow-ups to monitor patient progress, adjusting treatment approaches as needed based on ongoing assessments.
  • Create relapse prevention plans for at-risk patients, facilitating strategies to ensure stable functioning and improve quality of life.
  • Document patient progress thoroughly in treatment charts in accordance with agency policies, ensuring that all clinical records are up-to-date and accurate.
  • Educate patients on self-management skills and empower them with the knowledge necessary for active participation in their treatment and recovery.
  • Collaborate with clinic management to provide insights on behavioral health services, contributing to program evaluation and improvement.
  • Support patients in adhering to medical treatments initiated by primary care providers, offering coping strategies to manage any medication side effects.

Your work will engage a diverse patient population, addressing a range of mental health challenges. You will be instrumental in recognizing and managing the effects of mental health on individuals and families, advocating for holistic care and integration across services available within our clinic. Qualifications

The successful candidate will possess:

  • A Master’s degree in Clinical Counseling or a related field.
  • A current Licensed Clinical Professional Counselor (LCPC) license in Illinois.
  • Extensive knowledge of mental health assessments, treatment planning, developmental psychopathology, and therapeutic techniques.
  • Proven ability to formulate and implement effective treatment plans tailored to individual patient needs.
  • Excellent communication skills to build collaborative relationships with patients, families, and healthcare providers.
  • Strong organizational skills for maintaining accurate clinical records and documentation.
  • Adaptability to fast-paced environments and dedication to ethical conduct in clinical practice.
Preferred Experience

While not required, bilingual proficiency in English and Spanish is highly preferred. Previous experience in a community health setting or with multidisciplinary teams will be advantageous. Familiarity with DSM-5 diagnostics and behavioral health management will further enhance your ability to thrive in this role.

Compensation and Benefits

We offer a competitive salary range of USD 87,000 – 105,000 annually, commensurate with experience and qualifications.
