Position Overview

Mindful Steps Counseling Services is seeking a dedicated and experienced Licensed Master Social Worker (LMSW) to join our team in Chicago, Illinois. This full-time, remote position offers a unique opportunity to make a meaningful impact on the lives of seniors and their families by providing essential social services and support in a dynamic community health clinic setting.

You will collaborate with a multidisciplinary team of healthcare professionals, including nurses, caregivers, and other social workers, to coordinate services around the clock. Regular video conferencing and phone consultations ensure you remain connected, supported, and engaged with your colleagues and clients alike.

Key Responsibilities
  • Conduct thorough assessments of seniors’ psychosocial needs, taking into account emotional, mental, and social factors that may affect their health and well-being.
  • Collaborate with clients and their families to develop individualized care plans tailored to their specific goals, preferences, and cultural backgrounds.
  • Provide counseling and emotional support to seniors as they navigate life transitions, such as relocation to assisted living, loss of a spouse, or health challenges.
  • Advocate for client rights, helping them connect to community resources, benefits, and services that enhance their quality of life.
  • Conduct regular virtual visits to monitor clients’ well-being, assess their living conditions, and address any potential safety concerns or required interventions.
  • Facilitate effective communication among family members involved in the client’s care, mediating conflicts to ensure a cohesive approach to support.
  • Educate clients and their families about community resources, social support programs, and benefits that can foster greater social engagement and wellbeing.
  • Maintain detailed, accurate, and timely documentation of client assessments, care plans, progress notes, and interactions within our electronic health record (EHR) system.
  • Stay updated on current social work practices, regulations, and ethics to ensure compliance and provide the highest standard of care.
Patient Population / Client Focus

Your focus will primarily be on elderly clients, many of whom may be experiencing various challenges related to aging, such as chronic health conditions, social isolation, mental health issues, and the need for assistance with daily living activities. Your work will significantly contribute to enhancing their quality of life, promoting dignity, and ensuring they receive the necessary support and resources.

Qualifications
  • Master's degree in Social Work from an accredited institution.
  • Current and valid licensure in the state of Illinois as a Licensed Master Social Worker (LMSW).
  • A minimum of two years of professional experience in social work, particularly in settings serving older adults.
  • Sound knowledge of social work principles, aging-related issues, and available community resources that support seniors.
  • Strong assessment, intervention, and counseling skills, with a focus on empathetic client interaction.
  • Excellent communication and interprofessional collaboration skills, essential for working within interdisciplinary teams.
  • Technical proficiency in using electronic health record (EHR) systems and other relevant software applications.
Preferred Experience
  • Experience in a community health setting or working closely with geriatric populations is preferred.
  • Familiarity with resources specific to Chicago and surrounding areas that assist seniors would be beneficial.
