Key Responsibilities

  • Maintain timely and accurate documentation of clinical records, ensuring compliance with healthcare regulations and best practices.
  • Collaborate with a multidisciplinary treatment team, including physicians, nurses, and other healthcare professionals to ensure holistic care for patients and their families.
  • Provide education and support to patients and their families regarding the emotional processes related to grief and end-of-life issues.
  • Stay informed about advances in mental health care and palliative support practices to continuously improve service delivery.
Patient Population / Client Focus

In this role, you will support a diverse patient population facing serious illnesses, with a focus on those receiving hospice and palliative care. Your expertise will be vital in assisting patients in navigating complex emotional landscapes, fostering resilience and coping strategies during a crucial stage of life. Engagement with family members to provide support and education is also a key aspect of this position.

Qualifications
  • A Master’s degree in Social Work from an accredited institution is required.
  • A valid LCSW license in the state of Tennessee is mandatory.
  • Proficiency in employing solution-based psychotherapy interventions and familiarity with trauma-informed care approaches.
  • Strong communication and problem-solving skills, with the ability to build rapport with patients and collaborate effectively with a multidisciplinary team.
Preferred Experience
